Terra Firma has a dedicated full-time case manager to provide as much support as possible to the Unaccompanied Minors and Families seeking Asylum that we serve. Please find below a non-exhaustive list of internal and external resources that may be helpful in the care of UICs & families:

Since Terra Firma is embedded within the Bronx Health Collective, Terra Firma participants have access to all services offered by the health center. The Nutrition team offers breastfeeding and lactation consulting, nutrition consultations, an emergency food pantry and a monthly food box distribution.

We enroll all eligible patients in health insurance at their first visit. In New York state, all children under the age of 18 qualify for Child Health Plus under the state CHIP program. If the patient is older than 18 we use PRUCOL documentation to apply for Medicaid. 

Terra Firma's case management team works to ensure that the basic needs of the UICs and families we serve are met. This includes, helping to enroll in school, referrals to social service agencies and ensuring Terra Firma participants are connected to legal services.
